What to Bring

Each person must present a valid photo ID at the time of fingerprinting. Revoked, suspended or expired ID will not be accepted.

Examples of acceptable ID:

ONLY IF, you are obtaining fingerprints for the Department of Health and Human Services (DHHS) will you be required to provide a fingerprint authorization form from your employer/company. This form will include a 6-digit number specifically assigned to you that is required for processing.

How Long We Hold Records For?

Please know all electronic fingerprint submissions will be held for only 30 days! Therefore, we encourage each person to follow up with their prospective agency/company whom has requested the fingerprints.
